    The following information is a brief overview of our health services and policies.

    Emergency Cards are done electronically through the GENESIS Parent Portal in the FORMS section.  This is information goes to the NURSE.
    Please complete promptly. Include the following information so we can better care for your child. Specific health concerns----i.e. asthma, food allergies, bee sting reactions, migraines, severe seasonal allergies or long term health issues that may impact learning or participation in physical education. (This information should appear on the annual health update also).

    Annual Health Update is for the NURSE's office only. It is also done eletronically through the GENESIS Parent Portal in the FORMS section.  Please complete prior to the first day of school.

    Medication Administration Any medication that needs to be administered in school must be accompanied by a signed doctor’s order and a parent note. It must be in the original container. NO student is permitted to carry medication in school.

         ♦ Self-Administration of Inhalers or EpiPens - A doctor’s authorization and a parent note must be submitted to the nurse’s office to enable your student to carry and administer their own inhaler or EpiPen in school. An Asthma Treatment Plan is required for students that carry their own inhalers. Please make sure any medication container sent to school is labeled. A nebulizer is available for use during the school day.

    Screenings Routine screenings are completed annually as per state guidelines. Height, weight and blood pressure are completed annually. Vision, hearing and scoliosis screenings are completed on a rotating basis. Please contact the nurse if you have any concerns.

    Physicals Documentation of a physical by your private physician is helpful in keeping your child’s health and immunization record up to date. This should be submitted to the health office at least once while a student attends the middle school.

    Immunizations Students entering 6th grade in September (age 11 or older) are required to have one dose of the meningococcal vaccine as well as a Tdap booster if they have not had one in the past five years. These need to be completed prior to the first day of 6th grade.  An incoming 6th grader may be excluded from starting school if this criteria is not met prior to the first day of school.  Please contact the Nurse's office as soon as possible with any questions regarding this.

    Physical Education Exclusions Students are expected to participate in gym unless they provide the school nurse and physical education staff with a signed doctor’s note for exclusion of greater than 3 days. A parent note with a valid reason will suffice for exclusions of 3 days or less.
