Athletic Physicals

 

The New Jersey State Interscholastic Athletic Association mandates that each candidate for a place on a school athletic squad or team shall be given an annual medical examination by a health care provider.  This examination includes: Height, weight, vision, blood pressure, pulse; assessment/examination of the eyes, ears, nose, throat, neck and skin; assessment/examination of the chest/lungs, spine, abdomen, heart; and musculosketal and neurological exams.  In addition, a genital exam for male candidates will be performed to assess for hernia and descent/ presence of testes. Physicals are valid if performed 365 days from the start of the season’s first practice.  Physical update forms, completed by the parent, must be submitted for participation in subsequent athletic seasons.

 

It is strongly recommended that the student’s private health care provider (physician or nurse practitioner) complete this examination. Physicals must be documented on the New Jersey State athletic physical form. The school physician will review these medical reports for compliance with the requirements of NJAC 6A:16-2.2(h)5 mentioned above, which is based solely on the results submitted by the examining health care provider. Parents will be notified if the examination does not meet all requirements, and forms that have missing documentation will be returned for further completion.  Students who will participate in athletics must return these forms to the high school no later than 3 weeks prior to the start of the new season, in order to allow adequate time for review.
