JAMES CALDWELL HIGH SCHOOL – HEALTH SERVICES
Dear Parents:
As we start another school year, I would like to remind you of our school health services. Our goal is to promote health and prevent illness. If for any reason you do not wish to include your child in any of these services, please contact me.
1. EMERGENCY CARDS
As we are here to take care of your child in the event of an accident or sudden illness, we are sending an emergency card. This card is our link with you. Please notify the school immediately of any change of home or business number. Please have your child return the card to their Mod 1-2 teacher the first day of school.
2. PHYSICAL EXAMS
Physical examinations are mandatory for all new students, athletes, and those who are under 18 years of age needing working papers. In the event your child has had a physical exam during the past year, such examination will be honored upon presentation of a record of such examination. It is recommended that the student's health care provider complete these examinations. Physicals are recommended for all students one or more years during adolescence. All athletic physicals must be documented on the New Jersey Athletic Physical form which can be obtained in the nurses office or athletic directors office.
3. HEALTH SCREENING
Height, and weight are checked yearly by the school nurse for all students. All 10th grade students will be screened for vision and hearing, and all 9th & 11th grade students will be screened for scoliosis. Any student will be screened at parent/student request. Parents will be notified by mail if apparent defects are noted. If you have any questions or concerns about this annual screening, please call the school nurse.
