Dear JCHS Parents:

The following information pertains to James Caldwell High School Fall Sports activity.

               ATHLETIC PHYSICALS

 

All athletes are required to secure an annual physical examination BEFORE he/she may participate in any practice or competition.   Physical examinations conducted within one year of the first day of any sport’s first practice session are acceptable. 

 

All physicals must be documented on the State of New Jersey athletic physical form, which can be obtained from the school nurse, athletic director, or the main office.  The health history must be completed by the parent/guardian prior to the physical exam.  All athletic physical forms must be reviewed and approved by the school physician, so completed forms for FALL sports should be returned to the school nurse no later than July 25, 2008 to allow adequate time for review.  Incomplete or inaccurately completed forms will be returned.

 

            If your child had a physical completed for winter or spring sports and it was previously submitted, the parent will need to complete an update/permission form and submit it to the school by July 25, 2008.
