CHANGE IN JCHS ATHLETIC PHYSICAL PROCEDURES
Dear Parents of JCHS Student-Athletes:
This letter is written to all parents who have children who might be interested in participating in any James Caldwell High School interscholastic athletic team during the 2006-2007 school year. All athletes are required to secure an annual physical examination BEFORE he/she may participate in any practice or competition. We are attempting to smooth the process by suggesting that all James Caldwell High School athletes who intend to participate in a sport during the 2006-2007 school year have physical examinations conducted before the end of the current school year, if at all possible. Physical examinations conducted within one year of the first day of any sport’s first practice session are acceptable.
It is strongly recommended by the State of New Jersey that students should be examined by their own health care provider. Arrangements to have the school physician, conduct your child’s physical examination should be requested only if you have no family physician, or if your personal financial situation requires us to request his services. Students who wish the school physician to conduct their physical examinations must see Mrs. Moran, who will make the necessary arrangements.
Please be advised that, under certain circumstances, including heart murmurs, high blood pressure and/or vision problems, Dr. Paulina may refer the student for a more extensive physical examination.
All physicals must be documented on the State of New Jersey athletic physical form, which can be obtained from the school nurse, athletic director, or the main office. The health history must be completed by the parent/guardian prior to the physical exam. As all athletic physical forms must be cleared by Dr. Paulina, completed forms should returned to the school no later than July 30th to allow adequate time for review (there will be a box in the main office for athletic physical forms during the summer). Incomplete or inaccurately completed forms will be returned to the student.
